Reports

Reports, complaints, appreciations, suggestions and observations are a valuable contribution to building more efficient and citizen-oriented health services. The complaints management system contributes to this.

How to send a report

Alerts may be submitted:

• using the electronic form on the Azienda USL di Bologna website;
• by telephone or in person at the Public Relations Offices;
• using the form that can be inserted in the boxes provided in all facilities;
• by letter and fax, which may be addressed to the Public Relations Offices or, for the Directorate-General, to Communication and Citizen Relations, via Castiglione 29 - 40124 Bologna, fax number 051 658 4822. Communication and Citizen Relations can also be contacted at relazioni.cittadino@ausl.bologna.it.
• Attention: without authorisation to process personal data (see below), we will not be able to respond to your message.

Personal data

The Citizens' Relations Office needs the following information in order to be able to respond:

• first and last name;
• address;
• telephone;
• having read the information on the protection and processing of personal data, pursuant to EU Regulation 2016/679;
• declaration of authorisation to process personal data (included in the reporting form);
• delegation in cases where the facts described in the reports concern particularly sensitive and personal situations relating to another person, attaching a copy of the identity documents of the delegator and the delegate.

Response times

Citizens who make a written and signed report are entitled to receive a response within 30 days of receipt.
